Got it too.
In my first fortress I used to see hungry/thirsty/drowsy status in the health z-screen but it stopped after a few years, and I haven't seen them in any fort since.

(Could it possibly be from init options? I think it stopped when I set ECONOMY:NO and a few other things, I'll try it tomorrow.)

What's your chief medical dwarf's Diagnosis skill? Higher skills appear to make more conditions to be visible on the Health screen - perhaps a sufficiently low skill omits hungry/thirsty.

Thirsty and hungry states (drowsy is untested) are visible at health screen in 34.06 (CMD with skilled diagnossis).

Still in 0.34.11

I can confirm that I also had the "Minor" thirst/hunger showing up at the beginning, but now only the starving/dehydrated are appearing.

Frustrating because I like to easily be able to check on how hungry/thirsty my fort gets so I know when to start cranking out more booze/food.

I realized this may be intended behavior because the chief medical dwarf's diagnostics skill may be rusting over time. Perhaps if working as CMD increased diagnostics or at least prevented it from rusting this would no longer occur.

The same problem is present in 0.40.06. That is status of Hungry dwarves and animals doesn't show on the health screen, while status of Starving animals show. Also Thirsty dwarves apparently also are not shown. Don't know about Dehydrated ones.

Here in this save some animals are Hungry, some are Starving, only Starving ones are shown on health screen.

The thing is, being Hungry/Thirsty/Drowsy is a normal thing to experience temporarily. It's not a medical condition; it wouldn't make sense for a doctor to get involved. I don't think this is a bug. It only seems like a bug because people are using the healthcare screen in a way that wasn't really intended.
