I dont have a save to share as I retired and unretired the fort where I experienced this. is it possible for dwarves to slip on snow covered ramps? If so then that explains the 2 or 3 dwarves that fell into the caldera while trying to floor over it. It does not explain the dwarf that jumped off the bridge and into the magma moat. Or the miner that decided he would dig the channel right underneath himself and end up in a pool of magma...

Actually, miners deciding to channel right underneath themselves is a completely different issue.
