Summary0003053: Lodging a weapon into a wound makes it forbidden.
Descriptionits simply as that:

The mithral drow two-handed sword has lodged firmly in the wound!
The house priestess slashes The Deer in the right front leg with her {mithral drow two-handed sword} and the severed part sails off in an arc!

as soon a weapon is lodged it will be forbidden for all additional lines. you have to claim it manually, because if the combatant drops it it will lay around forever (swapping for higher quality weapon for example)

This happened even in 40d. I suspect the "forbid death drops" logic may be tripping up here, seeing a weapon located in the enemy's inventory and forbidding it when it was removed.

Rhenaya, just FYI, the "AWAITING UPDATE" tag means "awaiting update from reporter," not "awaiting fix by developer". For the latter purpose, just monitor the report.
